New Patients

For your initial appointment, it’s very helpful if you have the following items and have completed the forms below:

• A list of all medications you’re currently taking
• A list of medication allergies
• Prior medical history
• Copies of prior CT’s or chest x-rays (if you had these tests at Conejo Imaging, Thousand Oaks Radiology, Los Robles Hospital, or Rolling Oaks Radiology, we can access the images via the internet).
• Health Insurance information

Review the Hippa Form that you must sign as part of your visit: HIPPA

Text Message Policy

We currently do not use text messaging to notify patients of upcoming appointments. However, if in the future we begin utilizing text communications and you prefer not to receive these reminders, simply call us and we’ll notify you of all appointments by phone.
